Plan Changes Effective July 1, 2012:

More information will be provided to you in the mail by EssentialCare and Macy’s.

• Effective July 1, 2012, the Macy's Limited Benefit Program will be referred to as "EssentialCare." The program design for EssentialCare (administered by PAI) will be changing. Important plan overview and FAQs can be located here.
• This Prescription Drug Claim Form will replace the prior form.  (Prescription drug claim forms for purchases made after June 30, 2012 will now be mailed to PAI, not Caremark, for reimbursement.)
• First Health will be the new preferred provider network. First Health provides access to approximately 5,000 hospitals and over 550,000 doctors and health care professionals across 50 states and the District of Columbia.
