First Health is the preferred provider organization (PPO) network for ULTA. Your Explanation of Benefits will reflect the First Health name and logo.
There are two easy ways to find a provider or health care facility that participates in the First Health Network: 1) Visit First Health online or 2) Call First Health’s Customer Service line at 800-226-5116 between 8 am and 8 pm EST.
EssentialCare Voluntary Fixed Indemnity Medical Benefits plan is straight-forward and easy-to-use.
We believe that coverage for benefits should be easy to understand. With EssentialCare, you can see your benefits and know how much the plan will cover before you go for services. This plan is not a “major medical” plan. It is designed to help you pay for basic medical needs, not catastrophic events.
Advantages of Your Indemnity Plan
Reimburses a fixed fee for services.
Even if the cost of your service is less than the scheduled amount, you get the full amount paid to you. Example: A generic drug costs $4.00, and your scheduled benefit is $20 per prescription. Your payment will be $20.00.
Includes payments for Labs & X-rays.
When you visit your doctor, often you receive additional bills for diagnostic services like lab work and x-rays. This plan pays a flat benefit for each day you require services.
Includes Network Discounts.
Your plan includes discounted fee for services at Beechstreet network doctors and hospitals that can save you money. Many providers will file your claim for you, and you can choose to assign your benefit towards their bill.
Be sure to show your ID card to your provider, even if you have used your maximum benefits, because the network discounts still reduce your bill.
Your plan includes EyeMed Vision Network discounts.
Your plan includes discounts at Caremark pharmacies.
Inpatient and Outpatient Benefits.
The plan pays a per day rate if you are admitted to the hospital for 24 hours or more. This plan pays for both inpatient and outpatient surgery and anesthesiology, but note the benefit is lower for outpatient procedures. You’ll also receive benefits for ambulance services, emergency room, and even skilled nursing and physical therapy, if needed.
Intensive Care Benefits.
Please note that if you require Intensive Care, these benefits pay in addition to the Standard Care benefits.
